Why Brisbane families choose us

Brisbane handles more hospital discharges than anywhere else in Queensland, and many of the people we support come to us straight from the Royal Brisbane, the Princess Alexandra, the Mater or the QEII. Our registered nurses pick up wound care, bowel care, PEG feeding and complex medication management on day one, so nothing lapses between the ward and the lounge room. We coordinate directly with your treating team and your support coordinator, and we keep clinical notes somewhere your GP can actually see them.

Where we work in Brisbane

Our Brisbane workforce is spread deliberately so travel does not eat your funded hours. On the northside we cover Chermside, Nundah, Everton Park, Aspley, Stafford and Kedron. On the southside we cover Mount Gravatt, Carindale, Holland Park, Sunnybank, Coorparoo and Camp Hill. West we reach Indooroopilly, Toowong, Kenmore and Chapel Hill, and east we cover Wynnum, Manly, Cannon Hill and Morningside. Inner-city participants in Fortitude Valley, South Brisbane, West End and Newstead are supported from our city roster.

Support built for how Brisbane actually lives

A roster that works in a Queenslander on a 600 square metre block does not automatically work in a two-bedroom apartment in Bowen Hills. Before we quote anything we look at the real constraints: lift and loading dock access, visitor parking for support workers, bathroom turning space, and whether a hoist will physically fit through the door. Because most Brisbane suburbs sit near a busway or a rail line, we use public transport for community access wherever it builds independence, and reserve supported transport for the appointments where it genuinely earns its cost.

Getting started in Brisbane

Most Brisbane referrals are matched to a support worker within five business days, and urgent hospital discharges are usually faster than that. Bring your plan, or your interim plan, and we will tell you plainly what it can fund, what it cannot, and where you are being underquoted elsewhere. Call 0493 376 525 to speak to someone who actually works in Brisbane rather than a call centre, or ask us to come to you.
